\nED staff. Women report being dissatisfied with care received in the ED during an early
\npregnancy loss. Women report privacy, dignity, compassion, 3 sensitive care, and the
\nacknowledgement of a loss as being important themes surrounding early pregnancy loss. The
\nED\u2019s fast-paced environment and a possible lack of staff awareness to this problem are barriers
\nto providing holistic care to women experiencing miscarriage.
\nEmergency department nurses are ill equipped to meet the psychological needs of women
\nduring a miscarriage. Nurses in the ED are not formally trained in principles of perinatal
\nbereavement, including meeting the complex psychological needs of clients experiencing the loss
\nof an early pregnancy. Nurses may lack the confidence needed to care for these clients. The
\nproposed study facility is a busy level III trauma center, in the southeastern U.S., providing care
\nto around 100,000 clients per year and is the second busiest ED in project state. According to
\nqueried diagnosis codes surrounding miscarriage (in 2016-2017), the department sees over 200
\nclients annually, up to 27 clients a month, and the mean number of clients impacted each month
\nis 20. There is no existing early fetal loss protocol for this population or special training in the
\nED at this facility. The lacking protocol, training, and\/or awareness of the issue can result in
\nclient\u2019s psychological, spiritual, and holistic needs being unmet and leads to client dissatisfaction
\nwith care. The project study facility has a satellite ED. The satellite ED does not have the same
\ndirect, easily accessible obstetrical nurse support as the main facility ED.
\nNurses are in a position to provide more holistic care to these clients in the emergency
\nroom with a guiding framework for managing pregnancy loss in the ED. At the core of nursing is
\nrespect for human dignity and kindness; this ethical principle extends to the women and her
\nfetus, a sometimes unrecognized client. A framework or 4 protocol for care surrounding
\nmiscarriage can help meet the complex psychological, spiritual, and holistic needs of women
\nexperiencing a loss in the ED. A framework can also increase nurses\u2019 confidence in caring for
\nthis population. <\/span><\/p>\n
