      Develop a focused SOAP note, including your differential diagnosis and critical-thinking process to formulate a primary diagnosis. Incorporate the following into your responses in the template:<\/p>\n

      • Subjective: <\/strong>What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What is the duration and severity of their symptoms? How are their symptoms impacting their functioning in life? <\/li>\n
      • Objective: <\/strong>What observations did you make during the psychiatric assessment?\u202f <\/li>\n
      • Assessment:<\/strong> Discuss the patient\u2019s mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses with supporting evidence, and list them in order from highest priority to lowest priority. Compare the <\/span>DSM-5-TR<\/i> diagnostic criteria for each differential diagnosis and explain what <\/span>DSM-5-TR<\/i> criteria rules out the differential diagnosis to find an accurate diagnosis. Explain the critical-thinking process that led you to the primary diagnosis you selected. Include pertinent positives and pertinent negatives for the specific patient case.<\/li>\n
      • Plan:<\/strong> What is your plan for psychotherapy? What is your plan for treatment and management, including alternative therapies? Include pharmacologic and nonpharmacologic treatments, alternative therapies, and follow-up parameters, as well as a rationale for this treatment and management plan. Also incorporate one health promotion activity and one patient education strategy.<\/li>\n
      • Reflection notes:<\/strong> What would you do differently with this patient if you could conduct the session again? Discuss what your next intervention would be if you were able to follow up with this patient.\u202fAlso include in your reflection a discussion related to legal\/ethical considerations (demonstrate critical thinking beyond confidentiality and consent for treatment!), health promotion, and disease prevention, taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).<\/li>\n
      • Provide at least three evidence-based, peer-reviewed journal articles or evidenced-based guidelines that relate to this case to support your diagnostics and differential diagnoses. Be sure they are current (no more than 5 years old).<\/li>\n<\/ul>\n

        \nin for your appointment today. I’m going to be asking you some
        \nquestions about your history and some symptoms. And to get started,
        \nI just want to ensure I have the right
        \npatient and chart. So can you tell me your
        \nname and your date of birth? <\/div>\n
        SHERMAN TREMAINE:
        \nI’m Sherman Tremaine, and Tremaine is my game game. My birthday is November 3, 1968. <\/div>\n
        DR. MOORE: Great. And can you tell
        \nme today’s date? Like the day of the week,
        \nand where we are today? <\/div>\n
        SHERMAN TREMAINE: Use any recent
        \ndate, and any location is OK. <\/div>\n
        DR. MOORE: OK, Sherman. What about do you know
        \nwhat month this is?<\/div>\n
         SHERMAN TREMAINE: It’s March 18. <\/div>\n
        DR. MOORE: And the
        \nday of the week? <\/div>\n
        SHERMAN TREMAINE: Oh, it’s a
        \nWednesday or maybe a Thursday. <\/div>\n
        DR. MOORE: OK. And where are we today? <\/div>\n
        SHERMAN TREMAINE:
        \nI believe we’re in your office, Dr. Moore. <\/span><\/div>\n
        DR. MOORE: OK, great. So tell me a little bit about
        \nwhat brings you in today. What brings you here? <\/span><\/div>\n
        SHERMAN TREMAINE: Well,
        \nmy sister made me come in. I was living with my
        \nmom, and she died. I was living, and not bothering
        \nanyone, and those people– those people, they just
        \nwon’t leave me alone. <\/span><\/div>\n
        DR. MOORE: What people?<\/span><\/div>\n
         SHERMAN TREMAINE: The ones
        \noutside my window watching. They watch me. I can hear them, and
        \nI see their shadows. They think I don’t
        \nsee them, but I do. The government sent
        \nthem to watch me, so my taxes are high,
        \nso high in the sky. Do you see that bird? DR. MOORE: Sherman,
        \nhow long have you saw or heard these people? <\/span><\/div>\n
        SHERMAN TREMAINE: Oh, for weeks,
        \nweeks and weeks and weeks. Hear that– hear that
        \nheavy metal music? They want you to think
        \nit’s weak, but it’s heavy<\/span><\/div>\n
        . DR. MOORE: No, Sherman. I don’t see any birds
        \nor hear any music. Do you sleep well, Sherman?<\/span><\/div>\n
         SHERMAN TREMAINE: I try to
        \nbut the voices are loud. They keep me up
        \nfor days and days. I try to watch TV, but they
        \nwatch me through the screen, and they come in
        \nand poison my food. I tricked them though. I tricked them. I locked everything
        \nup in the fridge. They aren’t getting in there. Can I smoke?<\/span><\/div>\n
         DR. MOORE: No, Sherman. There is no smoking here. How much do you usually smoke? SHERMAN TREMAINE: Well,
        \nI smoke all day, all day. Three packs a day.<\/span><\/div>\n
         DR. MOORE: Three packs a day. OK. What about alcohol? When was your last drink? <\/span><\/div>\n
        SHERMAN TREMAINE: Oh, yesterday. My sister buys me a 12-pack,
        \nand tells me to make it last until next week’s grocery run. I don’t go to the grocery store. They play too loud of
        \nthe heavy metal music. They also follow me there. <\/span><\/div>\n
        DR. MOORE: What about marijuana? <\/span><\/div>\n
        SHERMAN TREMAINE: Yes,
        \nbut not since my mom died three years ago.<\/span><\/div>\n
         DR. MOORE: Use any cocaine?<\/span><\/div>\n
         SHERMAN TREMAINE: No,
        \nno, no, no, no, no, no. No drugs ever, clever, ever. <\/span><\/div>\n
        DR. MOORE: What about
        \nany blackouts or seizures or see or hear things
        \nfrom drugs or alcohol?<\/span><\/div>\n
         SHERMAN TREMAINE: No, no, never
        \na clever [INAUDIBLE] ever. <\/span><\/div>\n
        DR. MOORE: What about
        \nany DUIs or legal issues from drugs or alcohol? <\/span><\/div>\n
        SHERMAN TREMAINE:
        \nNever clever’s ever. <\/span><\/div>\n
        DR. MOORE: OK. What about any medication
        \nfor your mental health? Have you tried those before, and
        \nwhat was your reaction to them?<\/span><\/div>\n
         SHERMAN TREMAINE: I hate
        \nHaldol and Thorazine. No, no, I’m not
        \ngoing to take it. Risperidone gave me boobs. No, I’m not going to take it. Seroquel, that is OK. But they’re all poison,
        \nnope, not going to take it. <\/span><\/div>\n
        DR. MOORE: OK. So tell me, any
        \nblood relatives have any mental health or
        \nsubstance abuse issues?<\/span><\/div>\n
         SHERMAN TREMAINE:
        \nThey say that my dad was crazy with
        \nparanoid schizophrenia. He did in the old
        \nstate hospital. They gave him his beer there. Can you believe that? Not like them today. My mom had anxiety. <\/span><\/div>\n
        DR. MOORE: Did any blood
        \nrelatives commit suicide? S<\/span><\/div>\n
        HERMAN TREMAINE:
        \nOh, no demons there. No, no<\/span><\/div>\n
        . DR. MOORE: What about you? Have you ever done anything
        \nlike cut yourself, or had any thoughts about killing
        \nyourself or anyone else? <\/span><\/div>\n
        SHERMAN TREMAINE:
        \nI already told you. No demons there. Have been in the hospital three
        \ntimes though when I was 20.<\/span><\/div>\n
         DR. MOORE: OK. What about any medical issues? Do you have any
        \nmedical problems? <\/span><\/div>\n
        SHERMAN TREMAINE: Ooh, I
        \ntake metformin for diabetes. Had or I have a fatty
        \nliver, they say, but they never saw it. So I don’t know unless
        \nthe aliens told them.<\/span><\/div>\n
         DR. MOORE: OK. So who raised you? <\/span><\/div>\n
        SHERMAN TREMAINE: My
        \nmom and my sister.<\/span><\/div>\n
         DR. MOORE: And who
        \ndo you live with now? <\/span><\/div>\n
        SHERMAN TREMAINE:
        \nMyself, but my sister’s plotting with the
        \ngovernment to change that. They tapped my phone. <\/span><\/div>\n
        DR. MOORE: OK. Have you ever been married? Are you single,
        \nwidowed, or divorced?<\/span><\/div>\n
         SHERMAN TREMAINE: I’ve
        \nnever been married. <\/span><\/div>\n
        DR. MOORE: Do you
        \nhave any children? <\/span><\/div>\n
        SHERMAN TREMAINE: No. DR. MOORE: OK. What is your highest
        \nlevel of education? <\/span><\/div>\n
