Part A<\/b><\/span><\/span><\/p>\n

This presentation is <\/span><\/span><\/span>titled mental health screening, <\/span><\/span><\/span>and interventions for <\/span><\/span><\/span>Level two trauma centers <\/span><\/span><\/span>with a purpose to illustrate the need and <\/span><\/span><\/span>options for <\/span><\/span><\/span>health screening and interventions. <\/span><\/span><\/span>The question we should ask is, <\/span><\/span><\/span>why is there no mental health <\/span><\/span><\/span>screening or interventions <\/span><\/span><\/span>available in our local hospital setting, <\/span><\/span><\/span>to help identify patients <\/span><\/span><\/span>who are at high risk of developing <\/span><\/span><\/span>post traumatic stress disorder or <\/span><\/span><\/span>PTSD after being involved <\/span><\/span><\/span>in an acute traumatic accident. <\/span><\/span><\/span>Some of <\/span><\/span><\/span>the background mental health screening <\/span><\/span><\/span>is not being completed at <\/span><\/span><\/span>our local level two trauma center. <\/span><\/span><\/span>So identify patients who are at risk of <\/span><\/span><\/span>developing PTSD after being <\/span><\/span><\/span>involved in a traumatic accident. <\/span><\/span><\/span>Intervention options are not <\/span><\/span><\/span>available in the inpatient setting, <\/span><\/span><\/span>specific to trauma patients. <\/span><\/span><\/span>Patients are being discharged back <\/span><\/span><\/span>into the community without screening, <\/span><\/span><\/span>H later identified as <\/span><\/span><\/span>developing mental health issues <\/span><\/span><\/span>associated with their injury with lack of <\/span><\/span><\/span>resources again it’s <\/span><\/span><\/span>provided from the hospital. <\/span><\/span><\/span>Patients who are involved <\/span><\/span><\/span>in a traumatic event, <\/span><\/span><\/span>such as the motor vehicle accident <\/span><\/span><\/span>or fall are at <\/span><\/span><\/span>higher risk of developing PTSD. <\/span><\/span><\/span>Screening does need to be implemented <\/span><\/span><\/span>to capture patients at a higher risk of <\/span><\/span><\/span>developing PTSD trauma after being <\/span><\/span><\/span>died and start interventions <\/span><\/span><\/span>as soon as possible. <\/span><\/span><\/span>This topic needs to be addressed now because <\/span><\/span><\/span>our local level two trauma center <\/span><\/span><\/span>does not have screenings and interventions. <\/span><\/span><\/span>Additionally, less than 25% of level <\/span><\/span><\/span>one and level two trauma centers across <\/span><\/span><\/span>the United States screen for depression and <\/span><\/span><\/span>less than 7% for PTSD symptoms. <\/span><\/span><\/span>When looking at this, stakeholders <\/span><\/span><\/span>are Trauma Medical Directors. <\/span><\/span><\/span>This is important because <\/span><\/span><\/span>they need to be compliant with <\/span><\/span><\/span>American College Assurgon guidelines, <\/span><\/span><\/span>which recently released guidelines <\/span><\/span><\/span>for implementing mental health screening <\/span><\/span><\/span>specific to identifying patients <\/span><\/span><\/span>that are at higher risk for <\/span><\/span><\/span>developing PTSD induced from <\/span><\/span><\/span>the traumatic injury that they had. <\/span><\/span><\/span>As well as our important patients <\/span><\/span><\/span>and offering <\/span><\/span><\/span>this screening to them in the hospital. <\/span><\/span><\/span>Key factors are nursing staff <\/span><\/span><\/span>and the capacity to do <\/span><\/span><\/span>the additional screening along with <\/span><\/span><\/span>all the other duties that <\/span><\/span><\/span>they are responsible for in the hospital. <\/span><\/span><\/span>As well as social workers, <\/span><\/span><\/span>in developing a protocol <\/span><\/span><\/span>for brief inpatient interventions, <\/span><\/span><\/span>while patients are still in the hospital. <\/span><\/span><\/span>Then our Health Informatics Department <\/span><\/span><\/span>in helping <\/span><\/span><\/span>to build a screen questionnaire <\/span><\/span><\/span>and automated referral system. <\/span><\/span><\/span>Looking at evidence based options for <\/span><\/span><\/span>screening. The three that I picked. <\/span><\/span><\/span>The first one is an automated PTSD screening, <\/span><\/span><\/span>which basically pulls from the patient’s EMR, <\/span><\/span><\/span>and it collects data <\/span><\/span><\/span>with the patient’s history, <\/span><\/span><\/span>the suicide screening <\/span><\/span><\/span>that’s already being done. <\/span><\/span><\/span>Any past traumatic injuries that they have, <\/span><\/span><\/span>the current injury or t that they sustain. <\/span><\/span><\/span>Known mental health issues <\/span><\/span><\/span>and medications that they’re taking, <\/span><\/span><\/span>and based on that information that is <\/span><\/span><\/span>in the patient’s medical record, <\/span><\/span><\/span>all of this data is <\/span><\/span><\/span>pulled and gives a PTSD screening score. <\/span><\/span><\/span>The pros on that would not require <\/span><\/span><\/span>staff to have to <\/span><\/span><\/span>ask questions to the patient. <\/span><\/span><\/span>The cons to that would be <\/span><\/span><\/span>potential limitations <\/span><\/span><\/span>with the electronic health record that <\/span><\/span><\/span>our hospital system uses being able <\/span><\/span><\/span>to automatically pull that data successfully. <\/span><\/span><\/span>The second one is called <\/span><\/span><\/span>Injured trauma survivor screening <\/span><\/span><\/span>or IT S S. <\/span><\/span><\/span>In the pros to this, there’s just a simple, <\/span><\/span><\/span>yes or no questionnaire <\/span><\/span><\/span>consisting of nine questions. <\/span><\/span><\/span>This screening has been <\/span><\/span><\/span>one of the best practice options, <\/span><\/span><\/span>and has been shown to be <\/span><\/span><\/span>a stable screening tool studies <\/span><\/span><\/span>showing a PTSD predictability of 93.94%. <\/span><\/span><\/span>The con to that is just relying <\/span><\/span><\/span>on nursing staff to complete and <\/span><\/span><\/span>then patients not wanting <\/span><\/span><\/span>to complete or refusing. <\/span><\/span><\/span>The last screening option is <\/span><\/span><\/span>the patient health questionnaire or <\/span><\/span><\/span>the PHQ two screening question. <\/span><\/span><\/span>The pros to that is the first one is <\/span><\/span><\/span>just a quick question <\/span><\/span><\/span>two questions that the patient answers, <\/span><\/span><\/span>and if those are positive, <\/span><\/span><\/span>then it goes into <\/span><\/span><\/span>the PHQ nine screening questions. <\/span><\/span><\/span>The cons to that, relying <\/span><\/span><\/span>nursing staff to complete. <\/span><\/span><\/span>And then again, <\/span><\/span><\/span>patients refusing to complete. <\/span><\/span><\/span>Then as far as the intervention, <\/span><\/span><\/span>order the prost to this or positive screening <\/span><\/span><\/span>would automated referral generated <\/span><\/span><\/span>an electronic health record <\/span><\/span><\/span>by submitting an order for <\/span><\/span><\/span>the mental health interventions, <\/span><\/span><\/span>and whether that be a social workers <\/span><\/span><\/span>or inpatient and psychotherapy. <\/span><\/span><\/span>The cost of this would be if the EMR <\/span><\/span><\/span>is not able to generate the order based <\/span><\/span><\/span>on selected screening and or <\/span><\/span><\/span>the medical provider if they are having <\/span><\/span><\/span>to manually enter that. <\/span><\/span><\/span>It just leaves room for error if they do <\/span><\/span><\/span>forget to do it <\/span><\/span><\/span>before the patient’s discharged. <\/span><\/span><\/span>The recommendation for best <\/span><\/span><\/span>practice best options <\/span><\/span><\/span>is to implement <\/span><\/span><\/span>the injured trauma survivor screening, <\/span><\/span><\/span>just based on its ease of <\/span><\/span><\/span>answering the <\/span><\/span><\/span>simple yes or no questions along <\/span><\/span><\/span>with just the supporting data from <\/span><\/span><\/span>the American College of surgeons that it has <\/span><\/span><\/span>a high success rate of identifying <\/span><\/span><\/span>trauma patients that are likely to <\/span><\/span><\/span>develop PTSD from their injury. <\/span><\/span><\/span>Then best practice would <\/span><\/span><\/span>be to if their screening is positive, <\/span><\/span><\/span>an alert is generated <\/span><\/span><\/span>to the trauma provider to <\/span><\/span><\/span>order a trauma psychology consultation. <\/span><\/span><\/span>We would just have to work with <\/span><\/span><\/span>our local health matics <\/span><\/span><\/span>to inquire if this is an option. <\/span><\/span><\/span>If it can be automatically <\/span><\/span><\/span>generated or if it will <\/span><\/span><\/span>have to be entered by the provider. <\/span><\/span><\/span>Lastly to conclude being <\/span><\/span><\/span>the only local level two trauma center <\/span><\/span><\/span>here in the State of Idaho, <\/span><\/span><\/span>not being compliant with the new guidelines. <\/span><\/span><\/span>This is a huge need, <\/span><\/span><\/span>and it’s something that needs to <\/span><\/span><\/span>be implemented <\/span><\/span><\/span>soon to help identify patients. <\/span><\/span><\/span>At least just offer that screening to <\/span><\/span><\/span>them in the hospital. <\/span><\/span><\/span>A lot of the patients are <\/span><\/span><\/span>And as far as the care team and <\/span><\/span><\/span>the providers are focused on <\/span><\/span><\/span>the patient’s acute injuries, <\/span><\/span><\/span>getting them stable. <\/span><\/span><\/span>Oftentimes the mental health side <\/span><\/span><\/span>of it is not looked at. <\/span><\/span><\/span>As far as an example of <\/span><\/span><\/span>patient is that a motor vehicle collision, <\/span><\/span><\/span>and their injury is they are quadriplegic. <\/span><\/span><\/span>Everybody, including the patient is focused <\/span><\/span><\/span>on this life altering injury. <\/span><\/span><\/span>They won’t be able to walk again, <\/span><\/span><\/span>do their normal cares, <\/span><\/span><\/span>take care of family and patient, <\/span><\/span><\/span>take care of family members. <\/span><\/span><\/span>Which does affect their mental health. <\/span><\/span><\/span>And I think PTSD is just not something <\/span><\/span><\/span>on the forefront of patients <\/span><\/span><\/span>thinking that that is a possibility, <\/span><\/span><\/span>you know, 30 days after being discharged. <\/span><\/span><\/span>So I think it’s something that’s very <\/span><\/span><\/span>important that needs to be Good morning, everybody. <\/span><\/span><\/span>My name is Mofaro today. <\/span><\/span><\/span>I’m going to be presenting to you <\/span><\/span><\/span>my health policy analysis addressing <\/span><\/span><\/span>obesity in the Omaha Metropolitan area. <\/span><\/span><\/span>So we’re going to begin by saying <\/span><\/span><\/span>that obesity has been <\/span><\/span><\/span>a very severe health problem, <\/span><\/span><\/span>and it has great <\/span><\/span><\/span>consequences for individuals for <\/span><\/span><\/span>the society at large <\/span><\/span><\/span>and the health care system. <\/span><\/span><\/span>And obesity has a new line <\/span><\/span><\/span>which substantively increases <\/span><\/span><\/span>the possibility of chronic diseases, <\/span><\/span><\/span>such as type two diabetes, <\/span><\/span><\/span>cardiovascular diseases, <\/span><\/span><\/span>and some type of cancers as well. <\/span><\/span><\/span>The above analysis is <\/span><\/span><\/span>therefore vital in attempting to curb <\/span><\/span><\/span>the analysis or <\/span><\/span><\/span>the obesity epidemic and embracing healthier. <\/span><\/span><\/span>S that are going to be needed to address <\/span><\/span><\/span>obesity require <\/span><\/span><\/span>much more than just individuals <\/span><\/span><\/span>changing their lifestyles. <\/span><\/span><\/span>They need comprehensive strategies <\/span><\/span><\/span>from the public health policies, <\/span><\/span><\/span>community based interventions, <\/span><\/span><\/span>and systematic changes to <\/span><\/span><\/span>build an environment that <\/span><\/span><\/span>facilitates and supports a healthy living. <\/span><\/span><\/span>The problem statement question <\/span><\/span><\/span>invited an answer again is <\/span><\/span><\/span>continuously rising obesity rates <\/span><\/span><\/span>that affect our local population. <\/span><\/span><\/span>Obesity is a complex condition, <\/span><\/span><\/span>and it is influenced by numerous factors, <\/span><\/span><\/span>including poor diet, <\/span><\/span><\/span>lag of physical activity, <\/span><\/span><\/span>and environmental factors as well. <\/span><\/span><\/span>The following practical strategies that can <\/span><\/span><\/span>help to address obesity, <\/span><\/span><\/span>significantly can be the ones that will <\/span><\/span><\/span>help people have a quality life. <\/span><\/span><\/span>It reduces the quality of <\/span><\/span><\/span>life and life expectancy <\/span><\/span><\/span>and significant economic costs. <\/span><\/span><\/span>For example, improved <\/span><\/span><\/span>healthy food accessibility, <\/span><\/span><\/span>safe spaces to engage in physical activities <\/span><\/span><\/span>and can also help <\/span><\/span><\/span>in managing diabetes in the community. <\/span><\/span><\/span>So for the last decade, <\/span><\/span><\/span>now this rate has been <\/span><\/span><\/span>rising progressively <\/span><\/span><\/span>this obesity rate, I’m sorry. <\/span><\/span><\/span>With the present percentage <\/span><\/span><\/span>at most and more than 30% of adults <\/span><\/span><\/span>and more than 20% of children face obesity. <\/span><\/span><\/span>The prevalence of obesity puts <\/span><\/span><\/span>not only health and well being, <\/span><\/span><\/span>but also a heavy economic burden <\/span><\/span><\/span>on the healthcare system, <\/span><\/span><\/span>and there is no need <\/span><\/span><\/span>to overemphasize the agency and <\/span><\/span><\/span>importance of formulating strategies <\/span><\/span><\/span>for the obesity pandemic. <\/span><\/span><\/span>So the environment of <\/span><\/span><\/span>the country regarding the problem of obesity <\/span><\/span><\/span>represents a very diverse set of <\/span><\/span><\/span>stakeholders and <\/span><\/span><\/span>essential influencing factors. <\/span><\/span><\/span>These stakeholders include <\/span><\/span><\/span>obese individuals and <\/span><\/span><\/span>families struggling <\/span><\/span><\/span>with or suffering from obesity. <\/span><\/span><\/span>Strikingly, key contributors to <\/span><\/span><\/span>the rate of obesity in <\/span><\/span><\/span>society are multi faced <\/span><\/span><\/span>with access to health resources, <\/span><\/span><\/span>socio economic inequalities, and cultural, <\/span><\/span><\/span>dietary norms, all playing a role. <\/span><\/span><\/span>Awareness campaigns initiated to promote <\/span><\/span><\/span>positive health behaviors <\/span><\/span><\/span>are necessary as well. <\/span><\/span><\/span>So now the different <\/span><\/span><\/span>options that can be implemented, <\/span><\/span><\/span>and these are evidence based options to <\/span><\/span><\/span>combat the problem of <\/span><\/span><\/span>obesity in or community and many. <\/span><\/span><\/span>And these can include <\/span><\/span><\/span>education and awareness campaigns, <\/span><\/span><\/span>and these programs will aim to <\/span><\/span><\/span>create awareness about healthy lifestyle, <\/span><\/span><\/span>proper nutrition, as well as <\/span><\/span><\/span>regular activities <\/span><\/span><\/span>that can help reduce obesity. <\/span><\/span><\/span>Improved access to resources, <\/span><\/span><\/span>Another vital option is improving access to <\/span><\/span><\/span>healthy food and physical <\/span><\/span><\/span>activity opportunities, <\/span><\/span><\/span>supportive policies and regulations, texting, <\/span><\/span><\/span>sugar drinks, making school, <\/span><\/span><\/span>meals healthier, and regulating food. <\/span><\/span><\/span>Advertising may create <\/span><\/span><\/span>systematic changes that <\/span><\/span><\/span>facilitate more nutritious choices. <\/span><\/span><\/span>The best way under <\/span><\/span><\/span>recommendations that can help <\/span><\/span><\/span>our community with the problem <\/span><\/span><\/span>of obesity is through increasing <\/span><\/span><\/span>access to health resources <\/span><\/span><\/span>supported by supportive <\/span><\/span><\/span>policy and regulation. <\/span><\/span><\/span>O obesity is complex and multi dimensional. <\/span><\/span><\/span>Therefore, different approaches are <\/span><\/span><\/span>needed to find a solution. <\/span><\/span><\/span>The following are the keys <\/span><\/span><\/span>to overcoming these challenges. <\/span><\/span><\/span>And these can include the engagement of <\/span><\/span><\/span>stakeholders, ensuring adequate funds, <\/span><\/span><\/span>having policies change successfully <\/span><\/span><\/span>and education and resources to <\/span><\/span><\/span>ensure that <\/span><\/span><\/span>sustainable behavior is maintained. <\/span><\/span><\/span>Once active engagement of <\/span><\/span><\/span>such stakeholders is ensured, <\/span><\/span><\/span>culturally relevant interventions will <\/span><\/span><\/span>be developed and widely accepted. <\/span><\/span><\/span>The core of continuous assessment will be <\/span><\/span><\/span>monitoring and evaluating the strategies. <\/span><\/span><\/span>These stakeholders can work <\/span><\/span><\/span>collaboratively drawing <\/span><\/span><\/span>on their particular strengths <\/span><\/span><\/span>and resources to create <\/span><\/span><\/span>a supportive environment <\/span><\/span><\/span>that will promote healthy living, <\/span><\/span><\/span>as well as reduce the rate of obesity. <\/span><\/span><\/span>In conclusion, I just wanted just to go <\/span><\/span><\/span>over the things that we <\/span><\/span><\/span>went over about obesity, <\/span><\/span><\/span>how it is a very urgent public health issue, <\/span><\/span><\/span>and this needs to be faced globally, <\/span><\/span><\/span>as well as locally, <\/span><\/span><\/span>and a problem is overwhelming, <\/span><\/span><\/span>Mal as this can <\/span><\/span><\/span>only be treated by identifying and <\/span><\/span><\/span>addressing its underlying causes <\/span><\/span><\/span>within a program of <\/span><\/span><\/span>expanded health related resources. <\/span><\/span><\/span>The ultimate goal is <\/span><\/span><\/span>to build an environment that <\/span><\/span><\/span>supports a healthy living choice. <\/span><\/span><\/span>And this living and improves <\/span><\/span><\/span>our community overall health <\/span><\/span><\/span>as well as well being. <\/span><\/span><\/span>Addressing both individual behaviors and <\/span><\/span><\/span>systematic barriers addresses the causes so <\/span><\/span><\/span>that a sustainable impact incapacity <\/span><\/span><\/span>is realized and <\/span><\/span><\/span>the quality of life enhanced for <\/span><\/span><\/span>all members of the community. <\/span><\/span><\/span>Maybe periodic monitoring and <\/span><\/span><\/span>evaluation of these strategies will set <\/span><\/span><\/span>the platform for knowing <\/span><\/span><\/span>what works and what areas <\/span><\/span><\/span>require further techno engineering <\/span><\/span><\/span>for enhancement, <\/span><\/span><\/span>and as well as just having <\/span><\/span><\/span>a supportive policy <\/span><\/span><\/span>and comprehensive education. <\/span><\/span><\/span>By fostering a supportive environment <\/span><\/span><\/span>and engaging all stakeholders, <\/span><\/span><\/span>we can create lasting change that promotes <\/span><\/span><\/span>a healthy lifestyle and <\/span><\/span><\/span>reduces the prevalence of obesity. <\/span><\/span><\/span>Ultimately, improving our community, <\/span><\/span><\/span>especially here in Omaha, <\/span><\/span><\/span>Nebraska and our communities overall health, <\/span><\/span><\/span>as well as the well being <\/span><\/span><\/span>And this has been my
