The discussion is as follow for:Using Social Determinants of Health (SDOH) in conjunction with ICD-10-CM codes can significantly enhance the understanding and management of patient health and outcomes. This approach allows for a more holistic view of health that goes beyond medical conditions to include the broader social, economic, and environmental factors that impact an individual’s well-being. The problem is that patients with chronic conditions tend to have higher rates of hospital readmission.  Our goal is to reduce readmission rates for patients with chronic conditions by addressing SDOH factors that contribute to their health status.  We will solve the problem of hospital admissions by integrating SDOH by leveraging Z codes in ICD-10-CM.<\/span><\/div>\n
