\nGrace is a first-year nursing student undertaking clinical placement for HNN112. She has been
\nworking with the same preceptor for the past 6 shifts and has been observed to be able to
\naccurately measure vital signs. During her 7th shift, Grace is asked to go and complete a vital sign
\nassessment on Mr Roberts, a 72-year-old man admitted yesterday for chest pain for investigation.
\nMr Roberts has been stable on the shift so far, and his vital signs have been within normal limits.
\nWhen Grace measures Mr Roberts\u2019 vital signs, she obtains the following data:

\nBP: 90\/54
\nSp02: 96% on room air
\nTemp: 37.1\u00b0C
\nWhen Grace has completed the assessment, Mr Roberts asks for assistance to ambulate to the
\nbathroom. On the way to the bathroom, Mr Roberts reports feeling faint and falls, fracturing his
\nhip. On review of this incident, Grace was asked why she had not reported Mr Robert\u2019s new
\npresentation of hypotension to her preceptor immediately.
