The standard of care at the current time is a window<br \/>\nof 3-4 hours from onset of symptoms. The outcome of this project is to have the<br \/>\npatient achieve an increased likelihood of independence in ADLs at time of<br \/>\ndischarge. I used websites<br \/>\nsuch as NIH, AHA and ASA. The manual search was done at Sutter Health with<br \/>\nreading through policies and procedures related to the protocol and how these<br \/>\nduties are carried out. I reviewed the policies specific to the emergency<br \/>\ndepartment. She had experienced migraine-like headaches for the previous<br \/>\nthree weeks and got medical attention at an external hospital, where she<br \/>\nunderwent evaluation, including a non-contrast CT scan of the head and a lumbar<br \/>\npuncture, both showing normal results. During a shopping trip which marked her<br \/>\nfirst outing following clearance from the emergency room, she suddenly<br \/>\nexperienced a sensation of feeling &#8220;weird&#8221; and excused herself to the<br \/>\nrestroom, where her mother found her several minutes later with left-sided<br \/>\nparalysis and confusion. Paramedics were called, and they suspected the woman<br \/>\nwas suffering from a stroke. (AHAJournals,<br \/>\n2019) <\/span><\/p>\n<p style=\"line-height: 200%; cursor: auto; color: inherit;\"><span style=\"font-size: 12pt; line-height: 200%; cursor: auto; color: inherit;\">The<br \/>\nNational Library of Medicine has conducted research on emergency department<br \/>\narrival to needle time called \u201cdoor-to-needle\u201d time in patients with acute<br \/>\nstroke. (NCBI, NLM, 2023) The data was extracted and analyzed from the Get With<br \/>\nThe Guidelines Stroke Registry. This stroke registry not only looks at what<br \/>\ntime the patient was brought to the hospital (day and night), the arrival to<br \/>\nmedication time, time to CT and time to lab, but also whether there was a<br \/>\ndedicated \u201cstroke team\u201d to take care of the patient. What they found was very<br \/>\ninteresting. The median DTN (door to needle) time for the day shift was 37<br \/>\nminutes and the DTN for the night shift was 59 minutes. The DTN time was 36<br \/>\nminutes when the stroke team was present compared to 51 minutes when there was<br \/>\nno designated stroke team. Additionally, the website offers guidance on<br \/>\nobtaining stroke certifications for hospitals, detailing the various<br \/>\ncertification levels based on available resources. It also offers valuable<br \/>\ninformation on stroke types, recognizing stroke symptoms, and emphasizes the<br \/>\nimportance of having a well-educated hospital staff equipped with resources for<br \/>\noptimal patient outcomes. Additionally, there&#8217;s a dedicated section on stroke<br \/>\nsurvivors, highlighting how sharing their experiences aids in emotional,<br \/>\nmental, and physical recovery. Literature also<br \/>\nsuggests that early administration of thrombolytic medication, aimed at<br \/>\ndissolving clots in ischemic strokes, has a direct correlation with improved<br \/>\noutcomes and reduced deficits. Regular trainings are required to ensure that<br \/>\nhealthcare professionals remain up to date with the latest research findings<br \/>\nand advancements in stroke management.
