Final Project Health Behavior Change
\nProposal Components<\/span> <\/span><\/p>\n

There are two parts: <\/p>\n

1.     <\/span><\/span><\/span>The
\nResearch Proposal: think conference proposal or research grant. Write in a
\nstyle according to your expected audience. In this case, Academia driven so
\nthink theory and research driven. <\/p>\n

2.     <\/span><\/span><\/span>The
\nProgram Evaluation Plan: think community grant proposal or funding source
\njustification. Write in a style that is less academic driven. Think process and
\noutcome driven. <\/p>\n

Note that I will most likely not know which part you
\ncompleted first. You will no doubt have tons of repetitive information. Cut and
\npaste as needed and reword to suit the context of the proposal or plan. <\/p>\n

\nif any information\/narrative should be in outline or bulleted information. <\/p>\n

\u2022       <\/span><\/span><\/span>Graphs
\nand tables are fine. When in doubt, reference it! <\/p>\n

Part 1: The Research
\nProposal <\/span><\/h1>\n

Research Proposal components: <\/p>\n

1.     <\/span><\/span><\/span>Introduction: <\/b>Should include a purpose
\nstatement, that answers the following: <\/p>\n

a.       <\/span><\/span><\/span>Why
\nis this needed? <\/p>\n

b.      <\/span><\/span><\/span>What
\nis this significance\/why is this significant? <\/p>\n

c.       <\/span><\/span><\/span>What
\nis your hypothesis? (if given X, Y will occur) <\/p>\n

d.      <\/span><\/span><\/span>What
\nis the intervention? <\/p>\n

e.       <\/span><\/span><\/span>What
\nis the target behavior? <\/p>\n

f.       
\n<\/span><\/span><\/span>What will be the limitations of this proposal? <\/p>\n

g.       <\/span><\/span><\/span>What
\nare the assumptions of this proposal? <\/p>\n

h.      <\/span><\/span><\/span>What
\ntheory will be applied? <\/p>\n

i.       
\n<\/span><\/span><\/span>Define\/explain the theory and why you chosen it
\nfor this proposal.<\/p>\n

None of these should be answered with
\n\u201cbecause I had to for this course.\u201d <\/p>\n

2.     <\/span><\/span><\/span>Methods: <\/b><\/p>\n

a.     <\/span><\/span><\/span>Subjects\/Participants: <\/b>Describe the
\ndemographics of the subjects\/participants\/population.  <\/span><\/p>\n

\u2022       <\/span><\/span><\/span>How
\nmany, how old, what is special about them? <\/p>\n

\u2022      
\n<\/span><\/span><\/span>What criteria was used for inclusion\/exclusion? <\/p>\n

\u2022      
\n<\/span><\/span><\/span>How, where, and why were the subjects recruited?\n<\/p>\n

\u2022      
\n<\/span><\/span><\/span>Were there any rewards for participation given? <\/p>\n

b.     <\/span><\/span><\/span>Definitions: <\/b>are there terms which need
\nto be defined. For example, if you were to be applying a theory to increase
\nexercise, you would define exercise as you defined it for your participants
\nhere. Of course, a reference of some sort would be needed for that definition
\nor if you had your participants in your pilot define it, and you then used that
\nas your definition for your proposal\/project\u2019s working definition\u2014great! Just
\nsay that. <\/p>\n

c.     <\/span><\/span><\/span>Instrumentation\/Data Sources: <\/b>Survey?
\nObservation? What tool or technology will be used to collect the data? Is there
\na data collection sheet? (A sample copy of the actual data collection sheet
\nshould be included in the appendix). Will you introduce education? Will you
\npre\/post-survey or interview? Who developed the data collection method? Has the
\nsurvey been previously used and\/or published? Were the forms used piloted?
\nOther documents discussed here would be consent forms, permission slips, or
\nwritten permission from an organization to conduct the study\/project. <\/p>\n

d.     <\/span><\/span><\/span>Procedure: <\/b>Pilot, yes or not? What is
\nthe protocol or sequence of events? How long is recruitment? How did
\nrecruitment take place? Was there any training involved for data collectors or
\nfor participants? How was training developed? Place any training guides in the
\nappendix. <\/p>\n

This is your recipe of how, when and where
\nyour proposal happens. Imagine if someone wanted to reproduce exactly what you
\nare proposing, this section would give them a step by step of how to do what
\nyou are proposing. <\/span> <\/span><\/span><\/p>\n

3.     <\/span><\/span><\/span>References: <\/b>Listed in APA style \uf0e0<\/span><\/span>
\nminimum of 15 references needed for this proposal<\/p>\n

Part 2: The Program
\nEvaluation Plan <\/span><\/h1>\n

You may find it helpful to review Chapter 15 and 16 of the
\ntextbook to get grounded in what is involved in engaging with a population. <\/p>\n

Program Evaluation Plan Components: <\/p>\n

PARTS OF THE EVALUATION PLAN ARE ALSO ALREADY COMPLETED IN RED IN THE ATTACHED FILE <\/u><\/b><\/p>\n

1.     <\/span><\/span><\/span>Executive Summary: <\/b>Sort of an abstract.
\nThis is done after all other parts are completed. Usually a one pager that
\nsays, \u201cwe were asked to do this because ABC, so this is what we did, and this
\nis what happened. We do or do not suggest changes the next time something like
\nthis is done.\u201d <\/p>\n

2.     <\/span><\/span><\/span>Introduction: <\/b>Describe the overall
\nproject. For example, \u201cwe wanted to increase the stair taking at this facility
\nbecause our health insurance will give us a discount if we have wellness
\nactivities on site at the workplace.\u201d This is where you would describe an
\nentity or organization that the participants or subjects are coming from or why
\nthis program is important for this population. This is somewhat brief as you will
\nhave a purpose section after the needs assessment. <\/p>\n

3.     <\/span><\/span><\/span>Needs Assessment: <\/b>Describe how you
\ndecided on the target behavior you have chosen. Who did you talk to? What data
\nthat previously existed confirmed that you were matching a real need (data
\ndriven) with a perceived need (what the population or company or organization
\nwanted). <\/p>\n

4.     <\/span><\/span><\/span>Statement of Purpose, Program Goals, and
\nObjectives: <\/b><\/p>\n

a. <\/span>Statement of Purpose: \u201cthe purpose of
\nthis program is X\u201d <\/p>\n

5.     <\/span><\/span><\/span>Program Objective: <\/b>\u201cthe objective of
\nthis program is to ABC,\u201d think outcome. Describe how this objective was decided
\nupon. For example, \u201cafter a needs assessment of GHI and blah blah blah, the
\nprogram manager decided that HIJ objective would fit the company\u2019s goal while
\nmeeting the employee\u2019s needs.\u201d <\/p>\n

6.     <\/span><\/span><\/span>Intervention: <\/b>Describe what theory this
\nintervention is based on. Describe the theory briefly with an emphasis on the
\nconstruct you will be using or measuring. Describe the intervention for the
\nprogram. See Intervention section in the \u201cdo before you begin section\u201d at the top
\nof this document. <\/p>\n

7.     <\/span><\/span><\/span>Resources: <\/b>Are you getting help
\n(volunteers, data collectors, educators etc.) to accomplish this program? If so
\ndescribe any and all resources that will be assisting you. Maybe it\u2019s a
\nsituation where the city will be maintaining the park where your program is
\noffered. <\/p>\n

8.     <\/span><\/span><\/span>Marketing and Recruitment: <\/b>How will you
\nrecruit for participation? Look at the \u201cMethods\u201d section of your research
\nproposal. <\/p>\n

9.     <\/span><\/span><\/span>Implementation: <\/b>Describe what you are
\ngoing to do. Are there any concerns (legal, medical, recruitment, costs,
\nlogistics, recordkeeping, training, or safety)? List concerns and possible
\nstrategies in place or in planning to deal with these concerns. See the
\n\u201cLimitations\u201d section of your research proposal. <\/p>\n

10.  <\/span><\/span><\/span>Evaluation Plan: <\/b>Think Methods and
\nAnalysis, what will you do and why and how. Include data collection
\n(self-report, direct observation, existing records, meeting attendance). A
\nsample of evaluations will likely be a subset of your full target population so
\nif you have a population of 15 you may only evaluate 25% of randomly selected
\nparticipants to evaluate. Some evaluation plans will include all participants.
\nWhatever your plan is, justify it narratively. <\/p>\n

11.  <\/span><\/span><\/span>References (15): <\/b>If all references are the
\nsame (I don\u2019t think they will be but I\u2019m open to otherwise) you may simply
\nstate that the references are the same as the research part of your project.
\nEither part, research proposal or program evaluation plan, could have
