The optimal workflow requires having the right information at the right time so that the individual performing a step or task can advance the process toward completion. To achieve optimal workflow, organizations must take a step back and analyze the flow of work.
For this section of the project, you will work to collect data and know more about the current status of the organization. You need to know the readiness of the organization for this project. You also need to collect workflow information as early as possible, preferably before implementing a health IT system. You will also start building the project implementation plan.
I will focus on evaluating the impact of telehealth on hospital readmissions and disease management among patients with type 2 diabetes in rural communities. This narrowed topic addresses a well-documented and critical healthcare issue, as type 2 diabetes is one of the most prevalent chronic conditions and a major contributor to hospital readmissions. My project will explore how telehealth interventions—such as remote monitoring, virtual consultations, and mobile health apps—affect clinical outcomes (e.g., HbA1c levels), patient adherence, and access to care in rural settings where traditional healthcare services may be limited. I aim to assess both the clinical effectiveness and the system-level efficiency gained through telehealth, using existing peer-reviewed research and data from relevant studies. This focus will allow me to explore a clearly defined population, measurable outcomes, and the broader implications for improving healthcare delivery in underserved areas.

- Define the scope, objectives, and assumptions of your final project.
- Create a survey with at least 20 questions to learn about your organization’s readiness for this project. The survey should look into areas of end-user acceptance, leadership approval, business strategy, and the organization’s readiness and ability to implement HI initiatives.
- Create a flowchart using Visio® and analyze the current workflow. What changes are you planning to make to the workflow? How will the workflow be impacted after the implementation? How are you going to measure the improvements?
- Add the survey and the workflow analysis to the final project document and submit it to the Unit 4 Assignment Dropbox.
