2. Location and type of clients served specifically (age, gender)

3. Mission statement, What is the mission of your program?

4. For-Profit/or non- profit program.

5. Purpose/rationale-Why is this service needed in the CJ community? This information should be based on research from academic journals or research studies. Meaning you should find peer-reviewed articles to support the need for your program. You will need at least 3 peer-reviewed references for this project.

6. Treatment services offered specifically by your program.
