includes the title of your proposed study. For the title, you must rewrite your
central research question. For example, if your central research question is: How
can the problem of low test scores on the Ohio Educational Assessment for
Science be solved at Hampton High School in Ohio? Then, the title of your
study would be Recommendations for Solving the Problem of Low Test Scores on
the Ohio Educational Assessment for Science at Hampton High School in Ohio.

third page must be the references page and include the evidence cited in the
problem statement. Note: A references page may not be required if the evidence
