This is my PICOT question ” Among family practice healthcare providers in federally qualify healthcare center (FQHC) how does the implementation of evidence-based education on continuous glucose monitoring, compared to current practice, impact provider knowledge and patient hemoglobin A1C rate over Twelve weeks period.”My intervention is to educate providers (Nurse practitioners, PA, Physicians the use of CGM , to encourage them to increase or prescribe more CGM in their practice. This is the summary of problem identification. ” Diabetes disproportionately affects underserved populations, with many individuals experiencing suboptimal glycemic control due to barriers such as limited access to healthcare, inadequate education, and financial constraints. Continuous glucose monitoring (CGM) has been shown to improve glycemic outcomes; however, its adoption and impact in underserved populations remain underexplored. In this population, challenges such as limited health literacy and systemic inequalities may further hinder effective utilization of CGM technology. The plan is to solve the identified problem by educating FQHC healthcare providers on continuous glucose monitoring (CGM) in this population.
