COMPETENCIES

INTRODUCTION
Note: You must have completed and passed the performance assessment and clinical practice experience (CPE) for D159: Evidence-Based Measures for Evaluating Healthcare Improvements and you must have completed the CPE for this course before beginning this performance assessment.
After your first course, the remaining specialty courses throughout your capstone will each have a summative assessment that consists of an authentic performance assessment that scaffolds the tasks of the healthcare improvement project (HIP) through the project management phases of project initiation, planning, implementation, and evaluation. Each performance assessment will focus on aspects of the project management phases by requiring a real-world approach to improving healthcare with the implementation of a HIP. Each phase will be described in different sections of a HIP paper.
In this field experience course, you will apply the knowledge and skills you developed in previous courses toward the successful implementation of your HIP. This phase puts into action all the components of project management that you planned and developed while collaboratively working with key stakeholders to establish the need and feasibility of the HIP, strategically analyzing the organizational readiness for change, and planning the implementation and evaluation phases.
At the beginning of the project implementation, you will implement the training and communication plans for staff, managers, and leaders that were developed in previous courses. You will also experience managing the implementation process by applying organizational standards and practices. You will demonstrate strong leadership skills when meeting with stakeholders to report the status of the implementation phase and collaboratively address risks.
This task requires the submission of the attached “D160 HIP Paper Template,” including the following sections of your HIP paper, which you will be developing in this performance assessment:

• Control Phase (Monitoring) of Field Experience
This task requires the submission of the following which should be submitted as an appendix to your HIP Paper:
• SMART Goal Worksheet (completed in D156) (Appendix A)
During your clinical practice experience (CPE), you had the opportunity to submit a project journal and reflect on the NAHQ domains of healthcare quality. During your clinical practice experience (CPE), you will be evaluated on the evidence you submitted, indicating that you completed the required deliverables in your CPE record. The deliverables you produce in the CPE are evaluated for completion, not quality or content.
REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The similarity report that is provided when you submit your task can be used as a guide.
You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.
Tasks may not be submitted as cloud links, such as links to Google Docs, Google Slides, OneDrive, etc., unless specified in the task requirements. All other submissions must be file types that are uploaded and submitted as attachments (e.g., .docx, .pdf, .ppt).
Implementation of Field Experience
A. Evaluate the effectiveness of the process you used to kick off your HIP implementation, including one detailed example of each of the following components:
• organizing the project team
• building an alliance of supporters from the staff
• holding the kick-off meeting
• reviewing the timelines, goals, deliverables, and milestones with the team
B. Analyze the implementation of your initial communication plan developed during the planning stage of your HIP (D158). Include any modifications you made to the original communication plan during the implementation phase.
C. Analyze the implementation of your initial training plan developed during the planning state of your HIP (D158 v5- JAM2) or your PA (D160 v5 – JNM3). Include any modifications you made during the implementation phase.
Note: If you did not create a training plan in D158 because you were in v4 of the course, please create one now and reflect on it. Your training plan must include detailed information on the following components: Key topics, training method, training schedule, training outcomes (e.g., evaluations, test scores).
Note: You may submit your training plan in a table or narrative form for D160 v5 – JNM3 -rubric aspect C.
D. Explain how you tested the project implementation workflow. Include any modifications that you made during the implementation phase.
E. Provide two examples of how you integrated improvement science into your HIP implementation.
