Memoir Evidence-Based Diagnostic Assessment, Treatment Plan and Self Reflection. (150 points)
• Using the information from the Memoir book chosen, each student will present a case study following the assignment guidelines AND Rubric for Memoir Evidence-Based Diagnostic Assessment, Treatment Plan, and Self-Reflection details.
• Using the Diagnostic Assessment Template provided by the professor, student will complete a concise, detailed bio-psychosocial-spiritual assessment of the client from the Memoir. Student will provide a diagnosis for the client, including a detailed rationale for the diagnosis with client-specific behavior identified for each criterion. All sections of the Diagnostic Assessment must be thoroughly completed.
• Using the Master Treatment Plan Template provided by the professor, student will complete a client-centered treatment plan with goals and objectives relevant to the client’s diagnosis and presenting problem. Treatment plan must include a minimum of two goals and two SMART objectives for each goal.
• Include a two-page Self-Reflection Portion of content rich information that includes:
° Summary of feelings experienced by student while reading the Memoir;
° Discuss any anticipated or unexpected reactions;
° Explore how countertransference issues can be handled;
° Identify at least two self-care strategies that can help minimize compassion fatigue, vicarious trauma and burnout.
° A minimum of two scholarly research articles published in the last 8 years must be included to address concepts of countertransference, vicarious trauma, compassion fatigue, and burnout.
