Topic- Developing a Health Behavior Change Intervention: Final Project Part III—Evaluation & Resources/Organizational Information
You continue to work on your grant-style proposal detailing a behavior change program that you’ve designed, addressing an issue and target population of your choosing. In Part III, you are asked to write your program evaluation and resources and organizational information sections. A detailed program evaluation is vital for any program proposal and may include aspects of both formative and summative evaluation. It is also important to think about necessary resources to implement your health behavior change program. See the Assignment requirements for more information and areas to address in these sections.
Assignment -In approximately 2.5 pages, address the following:
- Evaluation: Describe the plan for monitoring progress, collecting key outcomes, and determining success. (approximately 1.5 pages)
- Resources & Organizational Information: Describe resources that would be required to implement the program (e.g., staff requirements, space, and equipment). Also discuss why you/your organization are well positioned to reach your target population and program partners. (approximately 1 page)
