This project will allow you to develop a grant-style proposal for a health behavior change intervention of your choosing, focusing on a specific target population in need of intervention, as indicated by existing data. Many health-related organizations—such as departments of public health, primary care associations, and healthcare clinics, to name but a few—compete for state and federal funds to implement programs designed to address behavior change. Depending upon your chosen career field, you may participate in such endeavors.
In this course, you do this by completing separate sections during Weeks 4, 7 and 9, which will then be combined to form a complete grant-style application, similar to what you might see in practice. In Part I, you are asked to write the introduction and needs assessment for your proposal, which provide background for your topic, your chosen target population, and the needs/behaviors it will address.
In approximately 2.5 pages, address the following:
- Introduction: Briefly describe the purpose of your proposed project (0.5 page maximum)
- Needs Assessment: Describe the target population and the need that the program addresses. Provide ample statistics and references to existing literature supporting the need for the program. Discuss existing resources (or lack thereof) and how your program will fill a specific gap/need. Tables and figures are welcome. Attach a separate references page at the end of the document. (approximately 2 pages)
attached is more info on the population I chose and the program I am looking to create please base this around that population and program
