   3. Data Collection: Develop a detailed plan for data collection, including data sources, collection methods, data management procedures, and ethical considerations.
   4. Data Analysis: Describe how you will analyze the collected data to determine the effectiveness of the program.
   5. Outcomes: Identify the specific outcomes you will measure, such as changes in knowledge, attitudes, behaviors, health indicators, or quality of life.
2. Improvement Plan: Based on your evaluation findings, select one area of your intervention that could be improved.
   1. Analyze Evaluation Findings: Thoroughly analyze the results of your evaluation, identifying both successes and areas for improvement.
   2. Prioritize Areas for Improvement: Based on your analysis, select one or two key areas of your intervention that could be improved to enhance its effectiveness or sustainability.
   3. Rationale: Provide a clear rationale for your selection, explaining why these areas are critical for optimizing the intervention’s impact and long-term viability.
   4. Improvement Strategies: Using evaluation findings outlines specific strategies for implementing the improvement, considering available resources, potential challenges, and stakeholder input.
