You are a third-year student nurse in a transition placement in a general practice when you
hear your RN preceptor on the phone discussing with a patient the results of another
patient’s positive chlamydia swab results. You see the phone call is documented in the
patients notes. Reading the notes, you learn that the nurse phoned to notify the patient
that he had been exposed to chlamydia. She also writes he is unable to come to the clinic
for treatment due to school commitments and a lack of transport. The nurse has written a
care plan that tells you the school aged boy needs to arrange a way to come to the clinic to
talk about the issue more. Several days later you enquire as to whether the patient has
made further contact or presented. You are advised to check his file and become more
aware of him. He is 14 years old and lives with his Nan. She is his documented NOK (next of
Kin). There is no further documentation evidencing him coming in or making contact with
any members of the general practice team .
