Read pages 1-20 and 152-161 of The Wellbeing of Somerville Report (2017). This is an excellent example of a community report card/community assessment!
Then, choose 1 age group and read in depth about the leading health indicators relevant to this group:
Birth-Age 4
Ages 5-18
Ages 18-24
Ages 25-39
Ages 40-64
Ages 65+
Within your chosen age group, pick one health indicator that you find interesting (e.g. access to healthcare, immunizations, behavioral and mental health, oral health, substance use disorder/addiction, physical health and development, education, economic stability, natural and built environment, social and community context, disability, etc).
Create a recorded presentation with voiceover using PowerPoint, Google Slides, or a similar format. If you use Google Slides, you must upload a downloaded copy; do not link to the web version! Make sure that your presentation includes the following:
Slide 1: Introduce your viewer to the city of Somerville and describe the overall goals of your presentation
Slide 2: Summarize the data on your age group and chosen health indicator. Include tables and graphics from the report as needed.
Slide 3: Based on the community data and recommendations provided by the Wellbeing Report, propose a community-level public health intervention for the city of Somerville. Be sure to include the supporting data (rationale) for your chosen intervention.
Slide 4: Write a SMART program objective for your intervention (see box 9.5 on pg. 201 in your text for help with this).
Slide 5: Describe your plan for evaluating the outcomes of your intervention
Slide 6: Conclude your presentation with a summary of the main points, including your leading health indicator, proposed intervention and evaluation method.
Slide 7: Include at least 5 peer-reviewed journal articles or evidence-based guidelines associated with your chosen health indicator or intervention.
