.As a Nurse Practitioner you will develop relationships with the patient, family, and other health care providers for optimal patient-driven health care.

Case Study

A 64-year-old widowed female presents to the office for a wellness exam. She is of Asian descent and foreign born. Her pap smears history is normal. She denies sexual activity for the past 5 years. Her last mammogram 1 year prior was normal. She has no other risk factors or family history for the development of breast cancer. She tells you she’s recently unemployed, as the factory where she worked over 20 years closed suddenly. She has no pension or retirement. She has taken Social Security income. She cannot afford national health insurance via the Affordable Care Act, and is hoping to just “get by” until her 65th birthday when she can qualify for Medicare. The patient picks up her teenage grandchildren from school three days a week. She has two adult daughters who live in town.

Address the following::

  1. How might your actions influence the relationship development with the patient, family, and other health care providers for optimal patient-driven health care in this case?
  2. How will you as the patient’s Nurse Practitioner guide this patient on what pap and mammogram screening is appropriate for her based on age and history?
  3. Are there other evidence-based health screenings that can impact the overall health of this patient?
  4. What interdisciplinary team approaches may be needed?
  5. What are some community-based resources that may be available to the patient?
  6. How would you address social support with this patent in her current situation?

This is a 2-3 page paper. Provide evidence-based sources.
