Candidates will:

- Choose and utilize one formal transition assessment and summarize results.
- Choose and utilize one informal transition assessment and summarize results.
- Create a minimum of four SMART transition goals based on priority need areas determined from the assessments.

- For each goal, create 3-5 instructional activities that are school and/or community-based to meet the student’s instructional and learning needs.
- List adult service/community agencies that provide supports in each need area identified. Describe how each agency can assist and explain the services that can be provided.
- Prioritize each of the goals from most to least important to address on a timely basis and build upon one another.
