Candidates will:

    • Choose a student with a disability to assess:
      1. Choose and utilize one formal transition assessment and summarize results.
      2. Choose and utilize one informal transition assessment and summarize results.
      3. Create a minimum of four SMART transition goals based on priority need areas determined from the assessments.

      1. For each goal, create 3-5 instructional activities that are school and/or community-based to meet the student’s instructional and learning needs.
      2. List adult service/community agencies that provide supports in each need area identified. Describe how each agency can assist and explain the services that can be provided.
      3. Prioritize each of the goals from most to least important to address on a timely basis and build upon one another.
