Technology

In this interview, I interviewed my neighbor who is a nurse Tatyana on a medical surgical/telemetry floor. She explained how it was scary working in the hospital during covid. She was inevitably exposed to it. She had a family of her own that she had to be cautious of making sure she is clean as possible to not bring it back home. PPE were limited. Patients here with one issue left with other issues or have gotten worse. It was interesting hearing her story as she worked as a nurse during covid. Ive always seen images and stories on the news, but hearing her personal story made me have a profound appreciation for healthcare workers who worked at that time. she explained how she had a variety of different patients. Some had a mild case of covid and were just being monitored and given fluids. Other cases were more severe, and more care and monitoring was necessary. Especially if they had a more extensive medical history such as obesity, hypertension, COPD, asthma. Those type of patients typically needed to be on antibiotics, steroids, nebulizer treatments, continuous oxygen monitoring, and/or bipap. She had around 5 patients daily since her hospital was smaller, but she said that it was definitely more stressful and higher acuity of patients during covid, but she is grateful for the experience since she has learned a lot of skills- especially of playing close attention to detail on patients for any changes. She mentioned how a patient was walking and talking in the morning. But as the day progressed, he was very lethargic, she called a rapid on him, and he ended up having a hemorrhagic stroke. I took this interview as a learning experience as well. As a new grad nurse, I have seen similar situations to what tatyana had explained to me, however working during covid must have difficult, stressful, and traumatizing.

Do they use a device, such as a blood pressure cuff, pulse oximeter, or glucose monitor, to monitor the problem?

Being in the hospital, there are a ton of medical devices used to monitor a patient. Tatyana said she mostly had patients who had a moderate case of covid and was there for monitoring. She did frequent monitoring on patients to make sure patients were stable and no new changes. They took vitals every 4 hours to monitor patients using the DinaMap, which is the machine to get vitals. Sometimes, requiring vitals to be monitored even more frequently depending on the patient. She always kept an eye on their oxygen and respirations to make sure they weren’t hypoxic or going into respiratory failure.

Tatyana mentioned that for patients who were progressively getting worse, she calls rapid responses. This gets doctors from different specialities to put their heads together to get help immediately. Typically, these rapids consists of vitals, bloodwork, EKG, connecting them to a heart monitor, and setting up the capnography. tatyana explained how before covid, she rarely used the capnography during rapids but because many of the rapids she called were regarding respiratory, capnography was used more frequently. It provides ventilation efficiency in current time and it measures end tidal co2 which measures CO2 at the end of each breath. It’s also used when patients have to get intubated or on ventilation. Tatyana said how patients who are ventilated go to the intensive care unit, so she never had experience with that, but there were a few times where patients had to get intubated immediately.

Have they used a smartphone app to help manage the problem?

Tatyana mentioned how many smartwatches can monitor heart rate and oxygen levels. She saw patients wearing smart watches monitor the heart rate and oxygen levels frequently which also helped her monitor and keep a baseline of the patient as well. She thought it was so smart, that unfortunately when her husband caught covid, she made her husband wear his smart watch to monitor.

Would telehealth be an option to help manage the problem?

Tatyana worked at the hospital where patients received care and treatment in person. However, she does agree that Telehealth is very helpful. It helps patients stay safe while also being proactive and getting the help that they need. Not only does it help with managing covid, but it helps to keep patients safe from getting covid. Also to avoid contracting any sickness, patients cancel or postpone their doctor’s appointments which means they are not getting the care or medical advice needed. Telehealth still provides safe access to getting proper treatment.

Tatyana mentioned that many patients return back to rehab or a senior home when they get discharged. Some patients from the senior homes do tend to come back

Have support groups been helpful in addressing the problem?

Covid definitely plays a huge role in the. Mental health as well as it can feel very isolating, especially sick in the hospital with very limited visitors if any. Tatyana said that during this rough time, she would try and connect with patients more with asking about their own personal life, drawing doodles on the white board. She also printed out different word puzzles to help the patient keep busy if they were up for it. She said many times the patients slept or used their own smart device to communicate with their family members.
