Part 2 Executive Summary: Enhancing Care for Veterans Through Patient Navigation

With the preceptor’s guidance, plan your evidence-based practice change and write an executive summary of the project: Enhancing Care for Veterans Through Patient Navigation. This executive summary should include a brief introduction and purpose of the project, background and significance of the project, theoretical plans(Theory and selected QSEN Competencies), the project’s plan with planned intervention strategies (Project plan and intervention strategy including timeline & budget detail ), an in-depth evaluation plan for assessing the project’s outcome, and Summary of impact to personal growth and professional practice. This executive summary should include multiple appendices that detail key components for implementing this evidence-based practice change.(See Attachments below for part 1 of the project)

This executive summary should be written as a professional APA manuscript-style paper. Your project will be an evidence-based practice change that is fully planned and developed, ready to implement in your practice setting. Please use one chosen theory and one or two selected QSEN competencies for graduate nurses to inform your project. For the selected theory, include a brief summary of the theory, its applicability to the project, and how this theory informed the planning of your intervention. Reflect on the QSEN competencies for graduate nurses and analyze how they informed your project. Describe how your work demonstrates achievement of these competencies. There is also a template available for you to follow as you complete the Executive Summary(See Attachment below). You should include all the elements of the template including any appendices listed. Use the rubric below to help you complete this project.

For the project you are proposing you need to develop a budget for the project. Part of the budget is to identify where the funding will come from for the project. Keep in mind you cannot use tuition or fees as these are already allocated to running the program.

1. All submissions should have a title page and reference page.
2. Utilize a minimum of five scholarly resources that are not more than five old from today’s date.
3. Adhere to grammar, spelling and punctuation criteria.
4. Adhere to APA compliance guidelines.
