Social determinants include housing conditions, transportation access, educational facilities/access, places of worship, social services, employment distribution, family income, etc. Physical determinants include history of the community, findings from your windshield survey, the built environment, the natural environment, physical boundaries/barriers, environmental conditions (air quality, toxic substances, sanitation, etc.). What are the determinants of health that affect your community of interest, and may be contributing to the health issue/s identified? • Research reputable internet sources (e.g. CDC, NIH) and/or peer-reviewed journal articles to determine 2-3 SDOH related to your health issue. • Outline the relationship between the SDOH and your health issue of concern (with citation from the literature) in 1-2 slides. • Provide additional relevant data for your community, a comparison community and California, related to each SDOH, using charts, graphs, etc. to demonstrate how each of these geographic areas is impacted the by your chosen determinants.
