The case management model of my choice is the Pennsylvania Department of Drug and Alcohol Programs clinical services manual; the population these manual serves are people who are diagnosed with alcohol and substance abuse addiction issues. This manual uses a comprehensive case management model; this is a process that helps clients address their needs for services and develop relationships to help with other issues, such as housing, unemployment, mental health, and domestic violence (Penzenstadler et al, 2017). The model provides a clinical care team, such as a counselor or therapist, who acts as both the case manager and connection to other clinical services; this model requires frequent contact between the case manager and the client.
Some evidence-based interventions they utilize includes substance abuse treatment programs, recovery housing, harm reduction, assessment and placement determination, level of care assessment, supervision, and educational programs. Some problems that may arise in this field is dealing with situations and services that the client needs, but the case manager is not trained in providing this service, therefore the case manager is responsible to outsource these services through another organization or recourse; the manual provides in depth information of what available resources that are located within the community and how to collaborate services.
The agency does not run into ethical or legal issues because it provides documentation for all services and treatment the client was referred to. The agency keeps strict adherence to patient confidentiality and updated policy and procedures relating to services provided, documentation on treatment ad outcomes, and updated patient information including consent to treatment and authorization to disclose any information.
