  1. Summarize your Community Assessment and Diagnosis paper submitted in Week 6 of NRSG 326 Community Health Nursing. See attachment
    • Make sure to identify the target population and main health problem.
  2. Literature Review
    • Conduct a literature review on the problem or risk.  The review should consist of three or more relevant scholarly journal articles.  See Literature Review Documents for tips.

  3. Plan (see Stanhope & Lancaster Boxes 14.2, 14.3 on pp. 255-256 and Box 14.4 on p. 258):
    • Develop one overall goal or purpose for a program that will help reduce the chosen health problem, concern, or risk and/or prevent the risk toward the targeted population.
    • Create three objectives to work toward achieving the above goal or purpose.
    • Develop a health education teaching plan to address the health problem, concern, or risk. Make sure to include the following:
      • Age-appropriate teaching strategies (see Stanhope & Lancaster, pp. 258-261)
      • Identify multimedia and/or technology used for teaching strategies (see Stanhope & Lancaster, pp. 262)
      • Describe collaboration or staffing necessary for the presentation
      • Document and explain the level(s) of prevention you are addressing with your teaching plan (primary, secondary, tertiary)
  4. Implement (see Stanhope & Lancaster, p. 248)
    • Consider how you will implement your health education presentation teaching plan and then answer the following:
      • Identify the location, best time to implement the program (day of the week, time of the day).
      • Examine at least two possible barriers for learning.
      • Identify at least two community agency services that you would refer learners for more information.
      • Describe how you will fund your plan.
  5. Evaluation (see Stanhope & Lancaster, p. 248, 263-264)
