For this portion of the database project, students will be comparing deficiencies among multiple healthcare facilities, and analyzing the data via a database program. Data will be presented in MS Access for manipulation into a Deficiency Report which will then require final screenshot submission via a Word Document. Students will then create a Data Dictionary for all columns as identified in the Deficiency Report. The data dictionary must be submitted via an Excel Spreadsheet.   

Two (2) Documents will need to be submitted.

You will be utilizing this database: HIT226DB.accdb

Deficiency Report: Access Database with 2 tables (40 points)

Identify 2 healthcare facilities to compare with 6 or more deficiencies

Instructions

MS Access Steps:

1. Open database in MS Access
2. Create new query using Query Design tool.
3. Add Providers and Provider Deficiencies tables to the query.
4. Add fields to query:
1. From Providers table: Federal Provider Number, Provider Name, Provider State
2. From Provider Deficiencies table: Provider Deficiency ID
5. Click Totals to change to an aggregate query
6. Under Provider Deficiencies ID
1. Change the Total setting to Count
2. Change Sort to Ascending
3. For Criteria, enter >= 6
7. Run the query.
8. Save the query as Provider Deficiency Counts.
9. Capture a screen shot of the first few rows of the query results and paste into a Word document.
10. Select 2 healthcare facilities from this list that you want to compare. Capture a screenshot including their Federal Provider IDs, Provider Names, Provider States and deficiency counts and then insert screenshot into a WORD document.

(Be sure your screenshot shows a minimum of 6 deficiencies per hospital)

Data Dictionary: Create a Data Dictionary (40 points)

Develop a data dictionary for all columns from your deficiency report in EXCEL. Your Data Dictionary must include the following data elements:

• Data field
• Name
• Definition
• Data Type
• Format
• Field Size
• Data Values
• Data Source

Please refer to Table 6.1 in Chapter 6 of your book for an example of a Data Dictionary.
