Develop a 20-question survey instrument for KEY INFORMANTS or for the TARGET POPULATION. Make sure that you have access to at least 25 people from the selected category to complete your questionnaire. Format all CLOSED-ENDED questions with the answer choices that respondents will have. You should have all closed-ended questions, with the exception of two, which should be OPEN-ENDED.  The two open-ended questions should address CAUSE (S) AND SOLUTION (S). In other words, you want to ask each person to say in his/her own words, what they see as the main causes of the problem and what they think can be done to solve the problem, or at least reduce the problem.  Also, if you have a look at the instructions for your NEED ASSESSMENT PROPOSAL you will see that you will have to use a second method in addition to the survey.

This should be a FOCUS GROUP, QUALITATIVE INTERVIEWS, or ANALYSIS OF PRE-EXISTING DATA.
